End of Life Care (Palliative Care)
Our teams are all contracted to provide support to individuals via the NHS Continuing Healthcare Fast-Track Pathway. This means that our teams are trained to support individuals with rapidly deteriorating health, and individuals who are nearing the end of their life. Our teams provide a sensitive and supportive service enabling individuals to receive the care they need to stay at home.
